Submit your application:

First open the pdf application, fill out the fields and print it.

Next, prepare a maximum of three pages that address the following areas, in the order listed:

  • General description of the project.

  • Describe how this project meets the Legacy Project criteria.

  • Project timeline. Give details regarding planning and implementation.

  • Describe the project location.

  • List project partners, if applicable.

  • Attach a proposed budget, showing total project cost and funding sources.

  • Please also provide letters of support from project partners and sponsors. These are in addition to the three-page maximum.

  • Mail the application and completed package to:

    Phoenix, AZ 85007
